Deaf Child Sign Request
Use this page to access the State Council for Persons with Disabilities (SCPD), former Council on Deaf & Hard of Hearing Equality (CODHHE) Deaf Child Warning Sign request. The form also provides information on the State of Need, Criteria/Procedures, and an Online submission form that is also printable to enable someone to print it out and assist in its completion for submission as an email attachment to return to SCPD@delaware.gov for processing.
